You will also need to print and fill out the Field Trip Permission Form(s) that pertain to your child. If you do not have access to a printer, you may come in the office and pick up a form.

FIELD TRIPS

Please print and fill out the Field Trip Permission Form that is applicable to your child or children, you will still receive notification of individual field trips.

STUDENT HEALTH INVENTORY FORM

You will need to fill out a student Health_Inventory.pdf form  for each child attending Thorp School.  If you are unable to print this form, all forms are available in the Thorp School office.
