Requirements for athletic eligibility:

• Code of Conduct Student/Parent Signature Form
• Concussion Form
• Current Physical
• Insurance Form
• Emergency Contact Form
• $25 ASB Card
• $15 Pay to Participate for each sport in the junior high
• $20 Pay to Participate for each sport in the high school

All students must be cleared for participation by Mrs. Crimp or Mrs. Pendley before they may participate in their first practice.
